Hours Vary JOB SUMMARY: The primary purpose of your position is to work directly with consumers in the implementation of the written habilitation plans, documenting consumers’ progress and response to the program services and ensuring the safety and well-being of the consumers. Generally works without direct supervision, works with individuals with more challenging needs and in home-based services. JOB REQUIREMENTS: Ability to lift persons if needed. Must be able to read, write and speak the English language. Meet licensing requirements of the state and county. Possess valid drivers license and maintain necessary insurance coverage. EDUCATION: Preferred to possess at a minimum, a high school diploma or equivalent and have successfully completed post high school education in a related field. NOTE: One year related work experience with successful completion of all related orientation and training requirements can be substituted for post high school education. EXPERIENCE: Must have one year experience working in the provision of habilitation services to individuals; a proven ability to work with individuals with extreme behavioral and/or physical needs. Desired additional experience: six months experience in the implementation of positive behavior management techniques; experience in providing support to families. Hours Vary
